Clinical Fellowship Program Requirements

If you are interested in our Clinical Fellowship Program in Cornea & External Disease to begin July 1, 2005, please complete and return the following:

• Application form (PDF file to follow)
• Brief (one page) statement of your professional goals, proposed objectives during the Fellowship, and details of particularly relevant previous experience.
• Photocopy of your certificate of registration for the December 2004 Ophthalmology Fellowship Match sponsored by the Association of University Professors of Ophthalmology for fellowships to begin July 2005.
• Score report for NBME or USMLE Steps 1, 2 & 3, or Flex both parts.
• Current curriculum vitae

Also, arrange for the following:

• College transcript sent directly to us from the institution.
• Medical school transcript sent directly to us from the institution
• Letters of recommendation should be sent by at least three professional associates (one to include your residency program chairman/director).

The deadline for completed clinical fellowship applications is August 31, 2004. Only completed applications will be considered for interviews.
